We opened our doors on June 1st, 2022 as part of 21st Century Rehab, PC. We are local to Iowa and therapist-owned.

Jake Cain
Clinic Director, Doctor of Physical Therapy
Jake was born and raised in Fort Dodge and has been a physical therapist in the area for 15 years. He is honored to serve individuals in the same community he grew up in. Jake is a member of the REC Board of Directors in Fort Dodge. In his free time he enjoys spending time with his family and friends, watching and playing sports, working out, and running and fishing with his German Shepherd.

Throughout Jake's years of experience, he has worked with a variety of patients with many different diagnoses. He has gained a great interest in those who have had athletic injuries as well as those who have undergone orthopedic procedures. Jake also has extensive experience with pre-employment testing/flex screens.

Tyson Comstock
Physical Therapist Assistant
Tyson is a Physical Therapist Assistant in the Fort Dodge area. Has been a PTA for 8
years working in outpatient rehab along with inpatient at the hospital. Has also spent time at the occupational medicine clinic doing pre work employment screens and flex tests. Loves spending time outdoors from morel mushroom hunting to kayaking and much more. Lives with his wife on his grandparents old hobby farm with animals. Has spent time working in the community at AFES, Brown’s shoe fit night and SIDS walk for the kids etc.

Taylor Illg
Doctor of Physical Therapy
Taylor was raised in Algona, Iowa and visited the Fort Dodge area frequently throughout his lifetime. He is a licensed athletic trainer in the state of Iowa. Taylor completed his Doctorate of Physical Therapy from Des Moines University in 2022 and is eager to move closer to come and help individuals reach their maximum physical activity potential. In his free time, Taylor enjoys spending time with his family and friends, catching up on his Netflix and Hulu shows, and golfing.

Over Taylor has done clinic work with multiple rural Iowa hospitals and has done a rotation with 21st Century Rehab in Carlisle during the summer of 2021. He specializes in sports medicine and orthopedics and has special interest in the areas of vertigo and lymphedema.

Nick Simpson
Doctor of Physical Therapy
Nick is a physical therapist at 21st Century Rehab, PC and Fort Dodge Physical Therapy. He is a member of the American Physical Therapy Association and the Iowa Physical Therapy Association. Nick’s was inspired to pursue a career in physical therapy after experiencing extensive rehab following multiple major knee surgeries as a child, as well as sports injuries. Nick enjoys golf, basketball, sand volleyball, and watching Kentucky athletics.
