Our story started just this year when two companies, 21st Century Rehab and Home Town Physical Therapy, teamed up to start a clinic in Fort Dodge. We are local to Iowa and therapist-owned.
We reduce and eliminate pain, increase mobility, and get people back to the activities they love through hands-on care, prescribed exercises, and patient education.

Jake Cain
Clinic Director, Doctor of Physical Therapy
Jake was born and raised in Fort Dodge and has been a physical therapist in the area for 10 years. He is honored to serve individuals in the same community he grew up in. Jake is a member of the REC Board of Directors in Fort Dodge. In his free time he enjoys spending time with his family and friends, watching and playing sports, working out, and running and fishing with his German Shepherd.
Throughout Jake's years of experience, he has worked with a variety of patients with many different diagnoses. He has gained a great interest in those who have had athletic injuries as well as those who have undergone orthopedic procedures. Jake also has extensive experience with pre-employment testing/flex screens.

Cole Jarrard
Doctor of Physical Therapy
Cole is a Fort Dodge native and is very grateful for the opportunity to return and serve the community with his expertise in health and wellness. After gaining clinical experience in Arizona, Cole was happy to return to Iowa and raise a family. He and his wife, Kelli, who is also a physical therapist, recently had their first baby. They are excited to be close to home, serving the Fort Dodge community!
Over the last 3 years, Cole has gained experience in a variety of treatment areas, including sports medicine, orthopedics, geriatrics and stroke. He has a special interest in helping those with head, neck, and TMJ issues and is specialized in the use of musculoskeletal ultrasound imaging.

Taylor Illg
Doctor of Physical Therapy
Taylor was raised in Algona, Iowa and visited the Fort Dodge area frequently throughout his lifetime. He is a licensed athletic trainer in the state of Iowa. Taylor completed his Doctorate of Physical Therapy from Des Moines University in 2022 and is eager to move closer to come and help individuals reach their maximum physical activity potential. In his free time, Taylor enjoys spending time with his family and friends, catching up on his Netflix and Hulu shows, and golfing.
Over Taylor has done clinic work with multiple rural Iowa hospitals and has done a rotation with 21st Century Rehab in Carlisle during the summer of 2021. He specializes in sports medicine and orthopedics and has special interest in the areas of vertigo and lymphedema.
